The Netherlands is a world leader in maritime technology. Many companies at the port of Rotterdam are dedicated to expanding the field. One of these companies is VSTEP Simulation, developer of maritime simulators and software for training purposes. In December 2020, Australian Pilot Captain Ritesh Bhamaria was expertly piloting oil tanker MT Godam through the Torres Strait in adverse weather conditions when he and the vessel crew sighted a distant hand waving for help in the rough sea.

Rostock; 02.05.2020: Actually, the special ship with the giant heavy-lift crane crane, which was designed for over 5,000 tons load capacity, was supposed to be delivered in the next few days. During final tests the crane broke off in the afternoon of May 2. Four people were injured in the accident. This is the second accident with Liebherr cranes this year in the Rostock overseas port. Already in February this year two cranes were reported to have fallen into the water during loading....

What happened?
MV "Shahraz" and MV "Samudra Sakti I" had run aground to the shallow Batu Berhanti in the Indonesian territorial waters within 6 minutes, which is surveyed with minimum water depths of 8.3 m in the northern part and up to 0.5 m in the southern part . It is located south of TSS Singapore Strait’s eastbound traffic lane.
A chain of events which must be described as extra ordinary rarity....

It has been shown that a majority (51%) of pilot ladders is secured by means of D-shackles, when secured at intermediate length. Previous research (Evans, 2020) has proven that this method has only about 50% of the strength of the pilot ladder when secured at full length (“double ended ladder”), or by means of the “endless-sling” method.
