To minimize the risk of a vessel allision with a terminal gantry crane, the American Harbor and Docking Pilots Association recommends that all terminal operators with gantry cranes adopt the following Best Practices.

Personal Locator Beacons (PLB) and other individual MOB devices have been available for some years now and have evolved to incorporate a variety of different alerting methods and combinations. However, usage of these additional enhancements within UK Pilotage is still in its infancy.

Going down the accommodation ladder with neither lifevest nor safety line must have been based on similar, past, successful operations, which may have never been challenged on board. This ‘slippage’ in safety is a pernicious and common phenomenon.

Correct method of rigging pilot ladder explained.
As explained, a large number of reports related to the use of pilot ladders which are too long relative to the vessel’s draughts. As a result, the excessively long ladders require shortening up before being deployed for boarding pilots. In these cases, ship crew shortens the pilot ladders, using D-shackles to choke the side ropes at the required height along the ladder’s length. Jacobsen Pilot Service and Los Angeles Pilot Station published a notice to all ships to inspect pilot ladders before the pilot boards.

Reason is an incident happened at the port of long beach in July 2020. Pilot ladder rope broke while the pilot was on the ladder - luckily the pilot was able to hold on.
