The international pilots association Impa is sounding the alarm about unsafe pilot ladders. According to the pilots, a worldwide survey has shown that access to the ship via the pilot's ladder often does not comply with the regulations.

On February 23rd, 2024 the NISSOS KEROS ( 330 m long x 60 m wide x 17m draft ) was inbound to Europoort, dolphins 90. Pilot was hoisted onboard by Noordzee Helicopters Vlaanderen (NHV) helicopter flying out of the Nederlands Loodswezen Rotterdam Pilots base. Assisted by 4 tugs from both Boluda and Muller, as well as KRVE boatmen, she eventually moored successfully in stormy weather on Maasvlakte-2 in Port of Rotterdam. U.S. Coastgard has published an update of their Recommendation for Pilot Transfer Arrangements in latest Marine Safety Information Bulletins (MSIB).

"Recent deaths of maritime pilots while embarking commercial vessels highlight the risks of operating in an
unforgiving maritime environment. To ensure the safety of all personnel boarding a vessel at sea, the Coast
Guard reminds vessel owners and operators of the requirements contained in the Safety of Life at Sea (SOLAS) Chapter V, Regulation 23 and strongly recommends that owners and operators follow the recommendations within IMO Resolution A.1045(27) – Pilot Transfer Arrangements. "
