Accident during embarking: Pilot's foot crushes

The UK Marine Accident Investigation Branch’s (MAIB) has published their latest annual report showing that pilot ladders remain a safety issue for workers.

As my colleagues know, IMPA conducts a “safety campaign” in the fall every year. The 2022 safety survey report was released a few days ago. First of all, we would like to thank the pilots who participated in this survey from all over the world.

An ATSB interim report outlines the sequence of events leading to the collision between the container vessel Maersk Shekou and the berthed tall ship STS Leeuwin II in Fremantle on 30 August 2024.

The Finnish special vessel "Meri" collided with Holtenau High Bridge (Holtenauer Hochbrücke) over the Kiel Canal early on 30 Nov 2022 morning at around 4.37 am.

Pilot ladder broke when pilot was boarding general cargo ship CREST CRANE at night Nov 5 at Keelung Port, Taiwan, to take the ship out.
