NTSB issued an investigation report on the contact of tow William C with a Rock Island railroad bridge protection cell, on Des Plaines River, in January 2020. The investigation established that high speed prevented the pilot to correct the tow’s position after completing the transit through the previous bridge.

Apparently there were two crew members on board, but no pilot. One of crew was recovered and declared dead, another one, skipper of the boat, is missing.

This is an exercise that is carried out at regular intervals by all crews on the Pilot boats and Tugs operating in Dublin Port, it is essential that crews are trained and ready to respond to any eventuality involving a MOB. In some instances it could be a casualty from another vessel. Today i tagged along on the pilot boat Pacific Pathfinder and her sister ship the Pacific Chinook to film their annual Man Overboard drills and re certification.
