Dredger OCEANLINE 5001 sailing at full speed struck anchored tanker STROVOLOS with moored alongside supply vessel, at around 1140 LT (UTC +8) Oct 2, on Melaka anchorage, Malaysia, Malacca Strait. The results can be seen on video – tanker suffered portside hull breach above waterline and apparently, some damage on cargo deck.

On August 5, 2020 at approximately 10:30 pm, Sandy Hook Pilot Captain Timothy M. Murray was involved in an incident while boarding a tanker vessel inbound to the Port of New York & New Jersey. He sustained injuries after falling from a pilot ladder and was evacuated to a local hospital where his injuries proved to be fatal.

The United Kingdom Maritime Pilots Association (UKMPA), is the representative professional body for Maritime Pilots in the United Kingdom. Almost every ship entering or leaving a port, is required under UK law to engage a duly Authorised Maritime Pilot, who either boards the vessel in port or at sea and takes conduct of the safe navigation of that vessel. Article NTSB investigation: Higher speed contributes to contact of tow with bridge

NTSB issued an investigation report on the contact of tow William C with a Rock Island railroad bridge protection cell, on Des Plaines River, in January 2020. The investigation established that high speed prevented the pilot to correct the tow’s position after completing the transit through the previous bridge.

Like many seafarers I have long been a keen follower of The Nautical Institute’s MARS programme, and along with many other members I listened to the recent webinar on that topic. One theme which was repeated more than once was that it is better to learn from someone else’s misfortune rather than have it happen to you. Having personally been involved in two near misses resulting from unsafe pilot transfer arrangements in a relatively short space of time, I asked how experiences and knowledge specifically about pilot ladder safety could best be promulgated to avoid repeating common accidents or near misses.
