Cargo ship crushes tugboat in Panama Canal

On 6 November 2017, the fully laden, mini cape-size dry bulk carrier Orient Centaur was transiting the South Channel, Weipa, Queensland, outbound under the conduct of harbour pilots as part of a trial introduction of this size of ship to the port. According to an article from "Safety at sea" the Korean Investigation Authority has published the first report about accident of container vessel "Milano Bridge" on April 6th 2020 in Busan.

Pilot boat with 5 people on board collided with breakwater at Kobe Port, Japan, at around 0300 Tokyo time Sep 4. 52-year old skipper of the boat, and 71-year old pilot, died in the collision, 3 crew were seriously injured.

NTSB issued an investigation report on the contact of tow William C with a Rock Island railroad bridge protection cell, on Des Plaines River, in January 2020. The investigation established that high speed prevented the pilot to correct the tow’s position after completing the transit through the previous bridge.

It has been shown that a majority (51%) of pilot ladders is secured by means of D-shackles, when secured at intermediate length. Previous research (Evans, 2020) has proven that this method has only about 50% of the strength of the pilot ladder when secured at full length (“double ended ladder”), or by means of the “endless-sling” method.

Here’s a nice video of the recent sea trials with ‘South Esk’, a new Interceptor 48 pilot boat for the Montrose Port Authority in Scotland. It was fairly rough during our sea trial days with a fair bit of wind, and although it was touch and go flying the drone, we managed to capture some wonderful video of her and Ederra 7 at the entrance to Cork Harbour in a gale.
